All rank one paper work and physicals must be turned into the Athletic Trainers

 

Dear Parents of an LISD Student-Athlete:

 

In an effort to stream line athletic paper work, Lewisville ISD is offering online forms. At the beginning of each school year it is mandatory that all student-athletes fill out ALL the appropriate UIL and LISD paper work before they will be allowed to practice or try out in athletics. This website is designed to stream line the process, allowing for less shuffling of papers. This new automated process will allow us to be more efficient in handling the forms as well as saving valuable resources. 

The following UIL and LISD forms can be completed online:
 

LISD:

Extracurricular Code of Conduct
Emergency Travel Card

 

UIL :

Acknowledgement of Rules
UIL Steroid Form
UIL Concussion Form
Cardiac Awareness Form
Medical Release Form 
 

Please take a minute and follow the directions to start this process of getting your student-athlete ready to participate. The actual hard copy of the Pre-Participation Physical must still be turned into the Athletic Trainer on campus. This piece of paper must be signed by the physician, parent, and student-athlete. You will not be allowed to participate if all forms are not filled out and Pre-Participation Physical turned in.
